Greenwich Village, NYC, Manhattan for the village Halloween Parade. It is AWESOME and lasts for hours. The restaurants are filled before during and after the event.

If you are into Haunted Houses, New Orleans has one of the best — The House of Shock run by Rob Zombie himself.
The French Quarter also has all types of people dressing up for the holiday. But the New Orleans celebration is more of the dark side of Halloween (ghost, vampires, ect). A trip on a Haunted Mystery tour would be an excellent Halloween activity.

The best in the US and possibly the planet is to see the Village Halloween Parade in New York City. This parade starts in the West Village and goes north through the west-side via the Avenue of the Americas. While best is biased and open for criticism this parade to answer your question is a must for anyone looking to have a great Halloween without your Arizona or Hawaii or anywhere else for that matter.
